R G Hospitals-Kolkata has done 09 MiLEP cases for enlarged prostate cases in last 15 days, using a 150 watt HOLMIUM laser for the first time in eastern India. The team of Urologists Dr.Amitava Mukherjee & Dr.Arindam Datta have done excellent work, which has instilled hope & admiration among patients.
1. Mr.Avinash Mitra (Name changed) 64 year Gentleman, a resident of Behala, Kolkata had undergone plastic Surgical repair of his urethra (urethroplasty) 17 years back. He presented with poor urine flow for the last 2 years, which did not respond to medical management.
He was evaluated and found to have a prostate enlargement of 73 grams. His urethra was generally smaller in diameter due to previous urethroplasty
He was planned for Laser Prostate Surgery, but during surgery, his urethra did not allow the routine Laser endoscope to pass through. However, the 22Fr Minimally Invasive LASER INSRUMENTS could be negotiated and he underwent MiLEP successfully with an uneventful recovery.
2. Mr.Suman Kumar Sen (Name changed) a 76 years Gentleman had previously undergone a routine TURP 14 years back. Over the last 4 years, he developed progressive prostate regrowth with reduced urine flow. Evaluation suggested a combination of stricture urethra due to previous surgery and prostate regrowth (BPH). In view of urethral stricture, it was decided to do MiLEP to prevent stricture recurrence. He had an uneventful surgery.
3. Dr. Bijoy Mukherjee (Name Changed) veteran Doctor needed prostate surgery for BPH. However he was very apprehensive about possible Urethral stricture due to the large endoscope and long catheter time. He was therefore offered minimally invasive Laser Enucleation of prostate (MiLEP) with small calibre endoscope. He had a small catheter placed after surgery which was removed after 22 hours.
At 2 months follow up after the surgery he continues to have excellent urine flow and has not developed stricture.
Speaking on the Press Conference Dr.Amitava Mukherjee –Sr.Urologist said- MiLEP is currently used on selected patients. The endoscope and accessory instruments are more delicate and fragile than routine LASER resectoscopes. However, it has the advantages of lesser stricture & shorter catheter times. MiLEP has the potential to replace HOLEP{ which is the current gold standard of prostate surgery} in the future.
